Statement of Purpose

The CPChem Human Resources Incident Reporting Form is intended for use by Human Resources to capture any incident that has been reported regarding potential violations of HR policies, unethical behavior, or an investigation. You may use the form if you are investigating a matter yourself, or if you require a neutral party to assist you with an investigation. Human Resources staff are encouraged to use this form to document matters which meet descriptions identified in the drop down list provided on the form when submitting an incident. The form should not be used to follow up on existing Ethics and Compliance matters.
